The Personal History needs to be completed prior to your first session. This form helps our staff to understand the events that have shaped your world view, which helps us pray for you and begin walking with you in your healing process. If timing permits, it is helpful to send the form in prior to your first appointment.

The Informed Consent form provides information about the ministry and practices. This form must be read and signed prior to your first appointment.

Prayers for Inner Healing are the prayers we commonly use in our ministry sessions. Some of these prayers are taken in whole or in part by similar ministries. We have included the processes we use and explanations behind the prayers that we use when discussing the purpose or reason for the prayer.
